Personal Care

Our personal care services can assist you with daily living activities, which you would normally undertake for yourself, but are unable to due to illness, disability or frailty.

We ensure you are provided with quality personal care services by:

Complying fully with the Care Plan developed between your case manager and yourself, treating medical practitioner and allied health professionals such as physiotherapist and occupational therapist

Ensure a safe, sensitive, private and dignified care environment

Working with you and your case manager to ensure the services you are receiving meet your needs and any changes will first be discussed with both yourself and your case manager

Assisting with daily living talks such as:
Bathing, and showering
Dressing and undressing
Shaving, hair care and grooming
Assisting with mobility and transfers
Assisting with rehabilitation and associated therapies
Continence support
Supervision of medication
Support with aids and appliances
Assistance with eating and drinking
Food preparation and accompanying you on community access and participation activities

In order to ensure a safe and healthy work environment we need to ensure that:

Your home has been assessed to be a safe work environment and our community care workers only do the work assigned to them

Your medication is provided in an approved aid such as a webster pack or dosette box; and

Your health is stable and does not require monitoring by medical personnel
